Tops Free Sports Screening Physicals

Student-athletes within GUHSD will have the opportunity to get cleared for the upcoming school year at no cost during the annual TOPS Free Sports Screening Physicals event.

The event will take place on Saturday, April 18, 2026, at Sunnyslope High School, located at 35 W. Dunlap Avenue in Phoenix. Participating schools have been assigned specific time slots, and student-athletes are encouraged to check with their coaches for their scheduled times. Walk-in appointments will also be available from 7:30 a.m. to 1:30 p.m.

These comprehensive physicals will be valid for the entire 2026–2027 school year and are required for participation in school athletics. All athletes must complete a new physical each year after April 1.

To participate, only official TOPS forms will be accepted. Forms can be obtained from coaches, school athletic departments, or downloaded from the TOPS website. Parents or guardians must complete the athlete information and medical history sections of the AIA physical form in advance. Additionally, the TOPS waiver and Page 5 of the AIA physical must be signed prior to the appointment.

Please review the following:

• No color paper, use black or blue ink, no double-sided printing.
• Do not fold/roll up the papers.
• Females: Please wear a sports bra under your primary garment.
• If athletes are wearing corrective eyewear, they must wear it for the exam.
• Immunization records are not needed.
• This is a 9-step process and could be 10 if the athlete needs an ECHO. (Check-up, vision, height, weight, blood pressure, EKG, physical, heart sounds, ECHO, and check-out with the doctor)

The free screenings provide a convenient and accessible way for families to ensure their student-athletes are ready for the upcoming sports seasons.
