All SPED Records request must be submitted in writing. You must include the following information for records to be released:

• Student's full name at the time of attendance 
• Date of birth 
• Attending campus and last year of attendance 
• Specify Special Education records are being requested
• Photo ID of Student, i.e Driver's License or State ID, must be included to verify identity
